Dreamflows places this site on the "North Fork Lewis" since that's the common usage in river running and allied circles, e.g. Bennett, AW, lewisriver.com, Pacificorp.
However, based on the gauge name, wikipedia, USGS topo maps, DeLorme Gazetteer, the National Weather Service, it's evidently officially just the Lewis.
Guess we must learn to live with the confusion. Just remember, when trying to locate the gauge or put-in/take-out points on the map, that it's really just the Lewis, not the North Fork Lewis.
